G.A. Family services is a founding member of CHHUNY, and a care management agency for Children’s Health Homes.
A Health Home is a unique care management model that is centered around your child and your family’s needs, strengths, and goals. CHHUNY contracts with numerous agencies throughout upstate New York to provide you and your family with quality care management services. A G.A. Health Home Care Manager can assist you and your family in many ways depending on your unique needs at any given time.
